CASE MANAGER
We’re recruiting an empathetic and proactive Case Manager to join a purpose‑driven welfare team supporting individuals through challenging circumstances. This rewarding role involves developing tailored support plans, coordinating resources and partnerships, and helping people access the assistance they need. Ideal for someone highly organised and compassionate, you’ll balance case management, stakeholder engagement and training delivery while making a genuine and lasting impact within a respected charitable organisation.

CASE MANAGER ROLE:
  • Working with clients to assess needs and develop tailored support plans covering mental health, financial wellbeing, employment, housing, family matters and welfare benefits
  • Managing and reviewing a caseload, monitoring client progress and adapting support plans to achieve positive outcomes
  • Assessing grant applications and coordinating with service providers to deliver sustainable solutions within the charity’s funding remit
  • Providing holistic case support, including signposting and referring clients to specialist agencies and support services where appropriate
  • Delivering training programmes, including Mental Health First Aid training, to industry organisations and stakeholders
  • Maintaining accurate and confidential case records, ensuring compliance with GDPR, reporting requirements and audit standards
  • Building and maintaining effective relationships with external agencies, charitable organisations and other key partners to enhance client support
  • Supporting the effective delivery and continuous development of charity services, including helpline cover, active reporting and attendance at relevant meetings and forums
CASE MANAGER ESSENTIALS:
  • Minimum of 12 months of experience working in social work or a mental health professional role
  • Holding a degree and/or relevant training in psychology, mental health or a related field
  • Working within a mental health, wellbeing or social care support environment
  • Using Microsoft Office applications, including Word, Outlook and internet-based systems
  • Managing exposure to distressing and emotionally challenging situations with professionalism and resilience
  • Demonstrating a respectful and collaborative approach when supporting clients and working with colleagues towards shared objectives and positive outcomes
